For referring physicians & healthcare professionals
Refer with confidence to a physician-led hyperbaric team in Scottsdale and Glendale. Your patients receive individualized care, and your practice stays informed.
Three straightforward referral options
Send referral details and attach your prescription or relevant records using your practice’s approved email workflow.
480-270-6094
Fax the referral, prescription if available, and your practice’s contact details for follow-up and chart notes.
Use the patient referral form below. Send prescriptions and additional records separately by email or fax.
A prescription is not required to start a referral. We are happy to evaluate your patient and prescribe HBOT when appropriate, or review and coordinate treatment under a prescription you provide. Every patient receives medical screening before treatment.
The standard your patients deserve
Use this checklist when choosing a hyperbaric referral partner. Your patients deserve a center that can document its equipment, training, clinical oversight, and treatment reporting.
Every RX-O2 chamber is FDA-cleared. Ask any receiving center to identify its equipment and document the clearance for the chambers your patients will use.
Patients receive 100% medical-grade oxygen through the prescribed breathing system. Oxygen delivery is part of the treatment plan and its documentation.
Every RX-O2 FDA-cleared chamber can treat up to 3.0 ATA. We review prescribed pressure, duration, frequency, and air breaks, and coordinate any clinically necessary changes with your practice.
Our technicians complete National Hyperbaric Safety Association training and supervised clinical experience in chamber operation, patient monitoring, and emergency procedures.
Medical Director Marvin A. Borsand, DO, FACOS, FACCS, provides medical leadership. Licensed clinicians evaluate treatment suitability, prescribe, and oversee patient care.
Our history of treating patients spans more than two decades, including experience with wound care, compromised grafts and flaps, and post-surgical recovery.
Monoplace and walk-in multiplace chambers help accommodate different patient needs. Tell us about mobility, positioning, or caregiver requirements so we can coordinate the appropriate clinic and chamber.
Your practice receives documentation of the care actually delivered, including treatment details and clinical notes, with patient authorization where required.
Make these standards part of your referral decision. Before entrusting a patient to a center, ask it to demonstrate its treatment capability and explain how your practice will receive the actual treatment record.

Clinical communication is part of the referral
As the referring provider, you should always be able to see what care your patient actually received.
We share treatment and chart notes with your practice, with patient authorization where required. Include your office contact information and preferred records-delivery details so we can keep the connection clear throughout care.
Documentation includes the delivered pressure, treatment duration, oxygen delivery, and air breaks, as applicable to the prescribed plan.
Your patient’s treatment tolerance, relevant observations, and chart notes help you follow the course of care.
We coordinate questions, concerns, and clinically necessary changes with the referring practice so the patient’s broader care remains connected.
Ask every referral center for this level of transparency. The treatment record should tell you what your patient received, not simply that an appointment occurred.
A referral process that fits your practice
Send the reason for referral, relevant medical history, and your contact details. Our licensed clinical team evaluates the patient and develops a prescription and treatment plan when HBOT is appropriate.
Choose the consultation-and-prescription option on the form below.
Attach your prescription when emailing, or include it with your fax. We review the ordered treatment and patient suitability, clarify any questions, and coordinate the course with your practice.
Use the form’s prescription-provided option, then email or fax the prescription separately.
Need a sample prescription or help with a protocol? We can provide the protocols we use and the supporting research, and connect you with our clinical team to discuss how they may apply to your patient.
